Data Request Form

Do you need data for PLCs, a degree requirement, a staff meeting, school improvement or other reasons? If you find that you need data, analysis of data, or program evaluation in one of those areas, you are in the right place! 

You can submit your request by using the Data Request Form link below. A cross-departmental group (Computing Services, Continuous Improvement, and Assessment & Evaluation) will review your requests and respond.

LPS employees, please use this form to request DATA you cannot find via Synergy or other data processes/sources (please check with others in your school/department to make sure there isn’t already a Synergy report or custom report that is applicable). If you are requesting support that isn’t specifically a DATA request (e.g. professional development, analysis or evaluation advice, how to run an existing report, etc.) please contact that department individually.
