High school department chair or middle school contact, in cooperation with the staff member responsible for the lab, completes this review monthly for each ITE lab. A copy of the results is given to the staff member responsible for the lab and a copy kept on file. Problems found should be discussed and a plan for remedy outlined on the next page.

Eye safety equipment is accessible and in proper repair. Other personal protective equipment is accessible.

Aisles, corridors, and access ways are free from obstructions.

Flammable materials are stored in approved storage cabinet and Used materials are properly labeled, contained and stored.

Equipment is in proper operating condition with all guards and safety devices in place and in proper working order. No equipment or guards have been modified.

Portable power equipment and hand tools are in proper working condition.

General "house keeping" is in appropriate order promoting a safe learning environment.

Eye wash stations or portable eye wash equipment is accessible and in proper condition.

Fire extinguishers and fire blankets are accessible. Fire extinguishers have been checked for proper charge within the past twelve months.
