The deadline to complete the Wellness Gift Card Proxy Sign Up has passed. Please make plans to pick up your gift card at the LPS District Office. Any cards not picked up by June 1st from the LPS Foundation will be considered a donation to LPS Wellness and will be utilized to fund future staff wellness efforts. Thanks for taking care of you!
Pick Up Dates/Times:
All cards will be distributed by the LPS Foundation from LPS District Offices at 5905 O Street.

- Wednesday, April 5th, 3:30-5: 30 pm
- Wednesday, April 12th, 3:30-5:30 pm
Weekend Options
- Saturday, April 1st, 9:00-11:00 am
