A suggested medical statement containing all required information is available here. Only licensed physicians may sign the medical statement for students with disabilities. It, or similar documentation must be kept on file for all students with disabilities receiving any special diet modifications. In most cases a short note from a physician on a prescription pad does not contain the required information. Required information includes:

    * the childs disability
    * an explanation of why the disability restricts the childs diet
    * the major life activity affected by the disability* and
    * the food or foods to be omitted from the childs diet, and the food or choice of foods that must be substituted.

How to Create A Web Payment Account

The LPS Meal Pre-Payment system allows district patrons the opportunity to purchase meals and deposit money into student meal accounts. Registered users may purchase meals or deposit money for all students in their family at one time and can do this with Visa, MasterCard, or Discover credit cards or by using your checking account information.

To use the system, you will need to create an account for yourself to access your students. In order to create an account you will need to have the LPS student ID for any one of your students in addition to that same student's birth date.
