Life Insurance Information

Lincoln Public Schools employees and their families have the opportunity to address their personal life insurance needs through either a term or permanent life insurance program. These programs are made available by UNICO Financial Advisors with coverage provided by Jefferson Pilot Financial for term life insurance and Security Financial Life Insurance Company for permanent life insurance.

NEW employees may enroll in up to $100,000 of term insurance during the first 30 days of employment without evidence of insurability. You need to complete an enrollment form that is available from HR. If you wish to apply for $150,000 or above (up to $300,000), you will need to complete a statement of health form (available from HR) and the enrollment form and submit both forms to HR.

Employees who elect term coverage may also apply for coverage on their spouse and/or children. You will need to complete an enrollment form and statement of health for each individual applying for coverage. Additional statements are available in the HR Benefits office.

During September, employees on staff may increase or enroll in $10,000 of coverage without evidence of insurability, to a maximum of $50,000. Fill out the enrollment form and submit it to HR. Employees on staff who wish to increase the amount of insurance they already have by more than $10,000 or employees on staff who don't have any term insurance now and wish to enroll in more than $10,000 must fill out a statement of health questionnaire and submit it with the enrollment form to HR.

Employee Rate: 19 cents per $1,000 of coverage. Please note: Term insurance ends on the employee's last day of employment.

Spouse Rates for Term Insurance
Rates are effective October 1, 2002 to October 1, 2005. Rates change on policy anniversary based on your attained age.

Children's coverage of $10,000 for all eligible children is $2.82 per month.
